For oral surgery and sedation dental practices

Your controlled‑substance log, without the end‑of‑day reconciling.

Controlled Ledger keeps the record a practice that stocks controlled substances has to keep: what came in, what each patient received, what was wasted and who witnessed it, and a count that has to match. Balances are computed from the entries, never typed, and the records an inspector asks for print in minutes.

What it records

One ledger per DEA registrant and per registered location, built around the federal and Ohio record rules each step serves.

Receiving

Stock received against a DEA-222, an electronic order or an invoice: date, supplier, reference, packages and units.

21 CFR 1304.22(c)

Administration and waste

The patient, the quantity given and wasted, the administering professional and the witness, recorded at the time.

21 CFR 1304.22(c) · OAC 4729:5-19-04

Counts

A physical count against the computed balance, with any discrepancy, its resolution and a second person's verification.

21 CFR 1301.76(b)

Losses

A shortage recorded with its evidence and your determination, the deadlines that follow, and a DEA Form 106 prefill.

21 CFR 1301.76(b) · OAC 4729:5-3-02

Disposal

Stock set aside, handed to a reverse distributor or destroyed on site with two witnesses, with a DEA Form 41 prefill.

21 CFR 1317

Inventories and exports

Initial and biennial inventories, Schedule II on its own document, and the full record as PDF and CSV for three years.

21 CFR 1304.04, 1304.11

History is never edited

A correction is a new entry with a reason, next to the one it corrects. Every action is written to an audit log that refuses changes. If a dentist approves what staff enter, that sign-off is recorded too.

What it is not

  • Not a clinical tool.No dosing advice and no clinical decisions.
  • Not a filer. It never sends anything to the DEA or the Board. Form 106 and Form 41 are prefills you complete and file.
  • Not a diversion detector. It shows discrepancies. Whether a loss is significant stays your decision.
  • Not tied to a distributor.Enter receipts by hand or from a simple spreadsheet file.

Price

$199per location, per month

Month to month. Every user at the location is included. Paid by invoice, ACH or card.
